Heavy Hands turns your output up. The slider runs from normal all the way to ten times, and whatever is in your hands hits proportionally harder - knife, crowbar, shotgun, all of it. At 2 or 3 it just takes the grind off. Up at 10, most things stop being a fight at all. It sits at 1 by default so nothing changes until you move it, and sliding it back hands your damage straight back to the game.

Does Heavy Hands affect melee weapons as well as guns?
Yes, everything you attack with scales together. In Nightmare of Decay the knife and the crowbar are what you fall back on when ammo runs out, and they are exactly where the padding is most obvious - a lot of swings for very little reaction. Heavy Hands lifts melee and firearms by the same multiplier, so switching to the knife stops feeling like a punishment. Between 2 and 3 keeps Nightmare of Decay recognisable. At that range enemies still take real hits and bosses still need a plan, but the grind of the fortieth identical swing is gone. Push toward the upper end of the slider and things fall over almost immediately, which is what you want for a Horde Mode run or for getting back to a section you actually care about. Because the Heavy Hands slider starts at 1 and returns there, you can experiment freely mid-run and hand the game its own numbers back at any point.
Does Heavy Hands make enemies hit me harder too?
No. Heavy Hands only scales the damage you deal in Nightmare of Decay - everything that attacks you does exactly what it always did, and your health bar takes the same hits it always took. That keeps the tension where the game put it while shortening the part most people find repetitive. If you also want to stop taking damage, Deathless holds your health bar steady while it is on and Field Medic refills it on demand. Running Heavy Hands alone is the lighter option: fights end faster, but they can still go badly wrong.
How does Heavy Hands change boss fights and Horde Mode?
Dramatically, which is usually why people install it. Nightmare of Decay's bosses are built around long damage windows and careful resource use, and its Horde Mode asks you to survive fifty waves of the same arithmetic. Raising the Heavy Hands Damage Multiplier collapses both - bosses drop in a handful of solid hits and waves stop outlasting your patience. If you want fights shorter but not trivial, keep the slider low and let the boss still demand positioning. Deep Mag pairs well when you would rather not think about ammo either.
