Does Spotless Record work from the main menu?
Yes. Spotless Record talks to Steam directly, so you don't need to load a save or be standing in the bunker. As long as Bunker Tidy Up was started through Steam and Steam is running, you can press Earn This Achievement or Earn Every Achievement from the main menu and the achievements arrive straight away. If Steam isn't connected, the mod tells you so instead of pretending it worked; start the game from your Steam library and try again. Earning achievements this way doesn't touch your save slots at all: your shelves, Points, upgrades and collectibles stay exactly as they were.
What happens if Steam turns down an achievement from Spotless Record?
Nothing is lost, and you'll know about it. Spotless Record asks Steam to confirm every award, so if Steam doesn't accept one, the notification says how many were turned down instead of claiming success. Press the button again to retry; achievements you already have are skipped, so retrying only goes after the missing ones. Right after the game starts, Steam can take a moment to send over your achievement data, and in that case the game can line the award up and grant it as soon as Steam is ready, and the mod tells you it's on the way. If Steam keeps refusing, make sure you're online and launched the game from Steam.
